The LLUH Institute for Community Partnerships is pleased to announce a call for proposals for Community Health Investment Awards. We invite nonprofits to submit project proposals aimed at addressing the needs of the most vulnerable community members in San Bernardino and/or Riverside Counties. Funding will be awarded to organizations that align with our priority areas, demonstrate a track record of strong community impact, and focus on improving health equity. We anticipate supporting projects that create significant and lasting positive change in our community.
To request funding an organization must:
- Be a nonprofit with required 501c3 documentation and tax ID.
- Serve people in San Bernardino and/or Riverside Counties or use the funds requested only to serve people in the two counties.
- Address one or multiple of the following priorities with proposed intervention:
a. Health Priority: Direct Mental Health Services
b. Health Priority: Maternal and Infant Child Health Initiatives
c. Health Priority: Heart Disease and Diabetes
d. Vital Condition: Meaningful Work and Wealth - Workforce Development
e. Vital Condition: Lifelong Learning - Scholarships
f. Vital Condition: Humane Housing
g. Vital Condition: Basic Needs for Health and Safety - Food Security & Basic Necessities
h. Vital Condition: Thriving Natural World - Sustainable Solutions & Access to Green Spaces
i. Vital Condition: Reliable Transportation
j. Vital Condition: Belonging and Civic Muscle
Deadline to submit: August 16th, 2024. Awards will be announced by September 27th, 2024.
